1. What is the purpose of this registration page?


The registration page allows Landmark members to access our healthcare data exchange platform. The platform facilitates the exchange of your Landmark clinical data with providers that you designate to receive it.

2. Who can register on this platform?


Members of Landmark Healthplan of California can use this portal to register an account. If you’re not a member of Landmark Healthplan of California but would like to be, please contact us.

Please note that the registering member must have been treated by a Landmark Healthplan acupuncturist or chiropractor and a claim submitted in respect of such treatment must have been paid. Claims are typically processed and paid within 30 days.

3. Is registration free?


Yes, registration on our platform is completely free of charge.

4. What information do I need to provide during registration?


You'll need to provide basic personal information such as your name, email address, date of birth, social security number, and contact information. Please note that the social security number and date of birth provided must be that of the subscriber of your Landmark benefit plan. The subscriber is typically an employee of the group that purchased the Landmark plan.

5. Is my information secure?


Yes, we take the privacy and security of your information seriously. We use industry-standard encryption and security measures to protect your data.

6. How will my information be used?


Your information will be used solely for the purpose of facilitating communication between you and the healthcare professionals that you designate on our platform. We do not sell or share your information with third parties without your consent.

7. How do I access my account after registration?


Once you've registered, you can access your account by logging in with the credentials you provided during registration.

8. What information do I need to grant access to my medical information to a provider.


Once you have registered and logged into your account you have the ability to grant access to your PHI inside the Landmark Health Plan of California network. In order to do this you will need the providers name, relationship to the patient, the provider’s email address, phone member, and address. Once this information is verified in our system and email will be sent to the provider to grant them access to the Landmark Health Plan of California’s Data Exchange Provider Portal.
