Landmark contracts directly with chiropractors and acupuncturists throughout California. Except in rare circumstances where the panel is closed, we invite chiropractors and acupuncturists to join our network. If you are interested in joining our network, Click Here.

Non Contracted Providers

Landmark may also offer coverage for non-contracted providers. To verify if a member has coverage to treat with a non-contracted provider, please contact our call center at (800) 298-4875.

Landmark also offers a secured provider portal, Landmark Connect, for providers to verify eligibility, claims status, and access administrative material. If you are actively treating a Landmark member and would like to register for Landmark Connect, please contact our call center at (800) 298-4875.

Dispute Resolution Process

You may dispute a claim or group of claims related to a post service claim by submitting a Claim Dispute Form within three-hundred and sixty five (365) days after receiving your claim denial. Click Here to download the Claim Dispute Form.
